What happens at an appointment

Our Certified Nurse Assessor will:

1.-. Perform interviews, collect health information from family, professionals or the primary doctor, and complete an assessment and the multi-page PRI and SCREEN documents.

2- Observe you or your elder loved one stand and walk. She will evaluate the level of cognitive clarity through questions and conversation with the elder. She may need to observe the skin and use of assistive devices.

3- All of this comes together to determine your loved one’s intensity of care needs and categorize them as heavy restorative, clinically complex, behavioral, or requiring physical assistance.
